What is Modifier 25?

Modifier 25 is appended to an Evaluation & Management (E/M) service to indicate that the patient’s condition required a significant, separately identifiable E/M service by the same provider on the same day as a procedure or other service.

In other words, if you treat a patient for an unrelated issue and then perform a minor procedure, modifier 25 tells the payer that both services were necessary and distinct.

Proper Use Cases for Modifier 25

Example 1: A patient comes in for a scheduled lesion removal but also presents with new symptoms of shortness of breath requiring evaluation. The separate E/M service would warrant Modifier 25.

Example 2: During a routine follow-up, a provider identifies an ear infection requiring immediate treatment distinct from the initial reason for the visit.

Common Mistakes with Modifier 25

Adding it Without Justification: Never append modifier 25 if the E/M service wasn’t distinct and separately documented.

Lack of Clear Documentation: The E/M note must explicitly describe the separate issue evaluated and treated beyond the procedure.

Using it for Pre-Procedure Evaluation: If the E/M is part of the decision to perform the minor procedure, modifier 25 isn’t warranted.
